Our Doctors of Physical Therapy complete an in-person functional assessment and provide the certification needed for your Michigan disability parking placard application, based on a real evaluation of your mobility, not just a form signature. Who qualifies for a disability parking placard in Michigan?
Michigan requires a licensed medical provider to certify that a mobility limitation substantially impairs your ability to walk — including conditions that limit walking distance, require an assistive device, or involve significant balance or cardiovascular limitations.
What does the certification process involve?
Our licensed physical therapist completes a hands-on functional mobility assessment in your home, then completes the required Michigan Secretary of State certification form based on the findings.
How long does the process take?
The in-home assessment typically takes under an hour. Once complete, you’ll have the signed certification needed to submit your placard application to the Secretary of State.
Is this covered by insurance?
Placard certification is generally a self-pay service, consistent with our concierge, cash-pay model — there’s no insurance billing or prior authorization delay involved.
