DISABILITY PARKING PLACARD CERTIFICATION

What Is Disability Parking Placard Certification?

A disability parking placard requires certification from a licensed medical professional confirming a qualifying mobility limitation. Our Doctors of Physical Therapy complete an in-person functional assessment and provide the certification needed for your Michigan disability parking placard application, based on a real evaluation of your mobility, not just a form signature. Placard Certification is part of ELEVATE Care Advisors, our Senior & Disabled Care Advisors program at ELEVATE Therapy Services, LLC.

Who qualifies for a disability parking placard in Michigan?

Michigan requires a licensed medical provider to certify that a mobility limitation substantially impairs your ability to walk — including conditions that limit walking distance, require an assistive device, or involve significant balance or cardiovascular limitations.

What does the certification process involve?

Our licensed physical therapist completes a hands-on functional mobility assessment in your home, then completes the required Michigan Secretary of State certification form based on the findings.

How long does the process take?

The in-home assessment typically takes under an hour. Once complete, you’ll have the signed certification needed to submit your placard application to the Secretary of State.

Is this covered by insurance?

Placard certification is generally a self-pay service, consistent with our concierge, cash-pay model — there’s no insurance billing or prior authorization delay involved.

Why Choose ELEVATE for Placard Certification?

  • Certification completed by a Licensed Doctor of Physical Therapy (PT), based on a real functional assessment.
  • In-home visits — no need to travel while managing a mobility limitation.
  • Fast turnaround with no insurance authorization delays.
  • Assessment findings can also inform broader Adaptive Equipment & DME or Senior Placement needs.
  • Coordination with your broader care plan through ELEVATE Care Advisors.
  • Licensed, concierge care from a provider who already understands Michigan’s certification requirements.

Individuals with a permanent mobility limitation

Long-term conditions that substantially limit walking may qualify for a permanent placard, renewed periodically.

Individuals recovering from surgery or injury

Temporary placards are available for mobility limitations expected to improve, covering the recovery period.

Older adults with reduced walking distance or endurance

Age-related decline in walking distance or endurance can qualify, even without a single diagnosed condition.

Individuals using a wheelchair, walker, or cane

Reliance on an assistive device to walk is one of the clearest qualifying criteria for certification.

Individuals with cardiovascular or respiratory limitations

Conditions that limit walking due to heart or lung function can qualify, even when mobility itself isn’t the primary diagnosis.

Caregivers transporting someone with a qualifying condition

The placard travels with the qualifying individual, helping whoever is driving them park closer and more safely.

Individuals renewing an expiring placard

Michigan placards require periodic renewal — we can complete the re-certification when it’s time to renew.

Veterans with service-connected mobility limitations

Veterans with service-connected conditions affecting mobility may qualify, and we can help coordinate with related VA benefits.
