What is AdaPTive Mobility Solutions?
AdaPTive Mobility Solutions provides comprehensive in-home assessments led by licensed Doctors of Physical Therapy. As movement experts, our therapists evaluate how medical conditions affect mobility and independence across the lifespan. Beyond physical therapy, we collaborate with occupational and speech therapy perspectives to address the full living environment—identifying barriers, recommending adaptive equipment, and promoting safer navigation of daily spaces. Our goal is to ensure that each individual can move confidently, communicate effectively, and live more safely within their home and community. AdaPTive Mobility Solutions is the dba of ELEVATE Therapy Services, LLC for its Senior and Disabled Care Advisory program.
What does a home safety assessment cover?
Our licensed physical therapists evaluate the full home environment: support available in the home, room layout, stairs, bedroom and bathroom accessibility, ADA/handicap accessibility, the garage, general safety concerns, and overall upkeep -- identifying barriers before they cause a fall or hospitalization.
What does the physical therapy evaluation include?
The PT perspective covers gross strength and range of motion, balance and coordination, functional mobility (transfers, walking, stairs, getting in and out of a vehicle), pain, and sensory factors like hearing and vision that affect safe movement.
What does the occupational therapy evaluation include?
The OT perspective covers activities of daily living -- dressing, eating and meal prep, toileting, hygiene -- plus advanced ADLs like housekeeping, driving, shopping, managing medications, and yardwork.
How do speech therapy and comprehensive planning fit in?
Our speech-language perspective addresses safety awareness, cognition, memory, and orientation. We also take a comprehensive approach to education gaps around insurance and available resources, caregiver support, and financial, legal, retirement, and mental health considerations that affect a safe plan for aging in place.

Individuals with disabilities from birth
We support individuals living with disabilities from birth, building assessments and recommendations around lifelong mobility and independence needs.
Fall risk assessment and fall recovery
We address both early causes of falls (mishaps and preventable hazards) and late causes (cognitive decline, vision changes, balance issues, weakness, or medical conditions), with a fall risk assessment aimed at reducing hospitalizations.
Medical conditions affecting mobility
This includes amputations and other medically vulnerable populations who need specialized mobility and safety planning.
Trauma recovery
We support recovery from orthopedic trauma (broken bones and pain), stroke, traumatic brain injury, spinal cord injury, and motor vehicle accidents.
Progressive conditions and home support transitions
For clients with progressive conditions, we help plan the transition into a more supportive home environment as needs change over time.
Financial considerations for aging in place
We work with low-income clients and financially savvy families alike, offering program awareness and guidance on which investments are worthwhile for aging safely in place, such as whether a stair lift or other modification makes sense for a given condition.
Family decision support
We provide unbiased, third-party recommendations on major decisions like home remodels and transitions, plus remote intergenerational monitoring so families have real answers about how a loved one is really doing.
Workplace accessibility and injury prevention
We also support workplace accessibility and injury prevention, helping clients stay safe and independent on the job.
