Senior Placement is a physical-therapist-led service that helps families identify the right living environment — independent living, assisted living, memory care, skilled nursing, or continued care at home — based on a real functional and mobility assessment, not just a checklist. As Doctors of Physical Therapy, our team evaluates how a person’s actual mobility, safety, and independence needs match what different care settings can provide, so families can make a confident, informed decision. Senior Placement is part of ELEVATE Care Advisors, our Senior & Disabled Care Advisors program at ELEVATE Therapy Services, LLC.

How does ELEVATE evaluate the right level of senior care?
Our licensed physical therapists assess functional mobility, fall risk, cognitive safety awareness, and the level of support available at home, then match those findings against the real capabilities of independent living, assisted living, memory care, and skilled nursing communities — so the recommendation is based on clinical evidence, not guesswork.
What levels of senior living should I understand?
Independent living (minimal support, social/amenity-focused), assisted living (help with daily activities in a residential setting), memory care (secured, specialized dementia and Alzheimer’s support), and skilled nursing (24/7 medical and rehabilitation care). How is a PT-led placement evaluation different from a typical referral service?
Many placement services match families to communities based on budget and availability alone, often paid by the community they refer to. Our recommendations come from a licensed Doctor of Physical Therapy’s hands-on functional assessment, giving you an unbiased, clinically grounded starting point before you ever tour a facility.
What happens after we choose a placement?
We support the transition with caregiver education, safe transfer and mobility training, coordination with the receiving community’s care team, and follow-up monitoring to confirm the placement continues to meet your loved one’s needs.
