A physician prescription is always welcome and helps us coordinate care — but is not required to begin Physical Therapy. Patients and families can also refer thåemselves. For all other services, simply call or fax.

AdaPTive Mobility Solutions
Sometimes a patient needs more than therapy — they need someone to help them navigate what comes next. Our licensed therapists serve as trusted care advisors for older adults, individuals with disabilities, and the families who love them — with a clinician’s eye and a family member’s heart.
Home Safety Evaluation
Clinician-led fall risk and accessibility assessment with written recommendations — ideal for discharge planning or aging-in-place concerns.
Wheelchair & Mobility Evaluation
Seating, positioning, manual and power chair evaluation, scooter fitting, and DME prescription support for new or transitioning users.
Adaptive Equipment Consultation
